Can you provide is with any more information on what's going on?  Could you attach the workflow.xml file you're using?

It looks like you're missing the Modification_GUID_FormUrl metadata entry that allows the framework to load the appropriate .ascx file.  Take a look at the sample shipped in the installer for an example of how to setup the Form Url.  If you have more questions, go ahead and let me know.
